It's something that goes beyond video-games altogether and permeates most media, speaking to what is permissible for men and women in society at large, as far as appearance is concerned. This is more than a rant just about BDO, because great difficulty in creating older, hard-faced, far from conventionally-attractive women is something I've encountered many times in video-games. One gets the feeling that any in-universe justification will be used by creators in general, to keep from having to create un-pretty women. BDO twists itself into a pretzel to justify why Wizards are old and white-haired, whereas Witches are young by default, by saying that women are naturally-attuned to magic in the setting and take less time to learn it. Want your Valkyrie to have a face that looks as if it's been used to hammer iron poles in to the ground, as is the case with the Berserker? Good luck with that. Want to make your witch a crotchety old crone? Good luck with that. Compare the Berserker or even the Wizard with any woman-only class and the character creation limitations become very apparent. They just end up looking odd and unnatural, because the base shape does its best to lock you into a soft-featured, tiny-nosed, doll-like mold. Worse, trying to make a middle-aged or even older woman is very difficult to do, if not impossible. They're far less distinct when it comes to both facial and body characteristics and rely on gear to set themselves apart across classes. are all varying shades of 'baby-faced', across all classes. The Wizard is old and weathered, with various options to make him look anywhere from a Dumbledore stand-in to a fey force of nature, the Warrior is young and fresh-faced, the Berserker has very strong, pronounced features and a hulking physique. The three man-only classes, Wizards, Warriors and Berserkers, are all very distinct characters, relying on far more than gear to distinguish them. BDO is a particularly egregious offender when it comes to this, due to how it distributes its appearance options across classes. I'll skip gripes about the gender-locked classes and get to something that infuriates me about character design in general: men can be harsh, gruff, even bestial appearances, but good bloody luck seeing the very same when it comes to women.
